Question One: How might the Hospice Nurse incorporate Interdisciplinary Care for the Patient?
The hospice nurse should help the patient understand the health care problem and the possible treatment plan. The nurse should follow up the questions by the patient and her family members. A pharmacist would also be of much help to teach the patient on how drugs work in her body, why she should take various medications and how to avoid drug interactions with other medications and food. The pharmacist will depend on the hospice care nurse to reinforce the information they have given to the patient (Wolfe, Hinds, & Sourkes, 2011).
Question Three: What Interventions might be considered?
The intervention of social workers in the care facility is also of much significance. They provide an important liaison between the hospice nurses and the community. They will help provide emotional support to the patient and her family members. Maximization of the interventions of the social workers is best when the hospice nurses are able to provide specific information about the home situation of the patient.
